Prepare for Your Visit

TOC understands your time is valuable and is committed to providing the best care to each and every patient. You will be seen as close to your appointment time as possible. There are a few factors that may cause delays in your appointment time such as high patient volume, emergency situations, and individual appointment times which vary by physician.

To make your wait time a more comfortable experience, we recommend you bring snacks, preferred reading materials, mobile devices and/or laptops. We offer complimentary Wi-Fi in all of our waiting rooms so you can stay connected while you wait.

Additionally, we recommend the following preparations which will help you make the most out of your time with your doctor:


  • If you are a new patient, complete and submit your medical history forms using our secure online Patient Portal.
  • Please complete, print and bring your Patient Forms with you to your visit.
  • Write down the date your injury occurred (if known).
  • Your doctor will want to know how your injury occurred and the symptoms you are experiencing. Knowing this information will help your doctor know how to best care for you.
  • Consider any differences to your injury or your health as a result of your injury.
  • Prepare and bring this list to your appointment.
  • Please plan to arrive 30 minutes early to find parking and complete any additional paperwork needed by your doctor.


  • Your photo identification.
  • Your up-to-date insurance information.
  • Method of payment to cover your co-pay or deductible.
  • Any MRI or X-Ray’s you have had (if any). You do not need to bring any MRI or X-Ray’s that were taken at TOC.
  • If you did not submit your medications through our secure Patient Portal, please bring a list of all medications you are currently taking, including how many times per day and the dose strength of your medicine. You may find it easier to bring in your medication bottles or packages for the doctor to see.
  • Your list of questions and concerns for your doctor about your injury.
