The fixed dollar amount set by your insurance contract that is required to be paid by you at the time of the visit.
The annual dollar amount set by your insurance contract that is deducted from insurance benefits and is required to be paid by you.
The percentage set by your insurance contract that is deducted from insurance benefits and is required to be paid by you.
The dollar amount to be paid by the patient who has no insurance benefits at the time of the visit.
The estimated amount calculated using your insurance benefits, contracted rates, and procedurers the surgeon plans to do. Final charges may vary if there are unplanned procedures and after your insurance processes the claim.
