This case report describes the first reported case of bilateral ischial tuberosity stress fractures in a 17-year-old high school football player presenting with nonspecific pelvic pain. Advanced imaging, including computed tomography, was used to establish the diagnosis and monitor healing throughout rehabilitation and return to sport. The report highlights the importance of recognizing uncommon stress fracture locations in adolescent athletes, as early diagnosis and appropriate management may reduce complications and facilitate recovery.
